Our Mission is to provide high-quality affordable healthcare in a more personalized and enjoyable setting. Our model is based on the Direct Primary Care (DPC) model.

What is the DPC model?

The DPC model is an insurance free model that allows providers to spend more time with patients for both face-to-face and telehealth visits. Patients and providers have an agreement with one another. Patients pay the practice a flat monthly or annual membership fee. The membership fee covers unlimited routine visits, an annual wellness exam and annual laboratory tests.

Our Goal

Our goal is to improve and maintain patients’ overall health by empowering them with knowledge and understanding of their condition. We offer a broad array of services geared to address today’s most common chronic health conditions and non-emergency injuries. We understand that there is no such thing as a one-size-fits-all cure, so we will never use a one-size-fit-all approach when managing our patients care.

Fusion Direct Medical and Wellness is the culmination of Co-Founders Yudelky and Sherita’s years of medical experience and hard work. Yudelky and Sherita are committed to providing accessible and affordable healthcare to all segments of society; they believe in the power of a strong Provider-Patient Relationship.
