Insurance and Billing

We participate with many different insurance plans and will file for reimbursement. It is your responsibility to know the details of your health insurance plan regarding referrals, co-payments and percentage of coverage. We recommend that you check your insurance benefits to be sure we participate in your plan.

TO OUR PATIENTS WITH MEDICAID COVERAGE, we only accept Amerigroup and WellCare (PEACHSTATE IS NOT ACCEPTED). Our office must be notified of any additional insurance you may have at the time of your visit.  Primary insurances must be billed first. Failure to provide this information may result in untimely filing and the patient will incur all financial responsibility.

  • We DO NOT participate in Medicare.
  • We Currently do not accept Care Source or Peach State.

Laboratory/Pathology Billing

All laboratory and/or pathology procedures are billed separately from your office visit. You may receive a laboratory bill from the following:

  • LabCorp: 1-800-845-6167
  • Quest: 1-800-326-4756

We require ALL payments at the time of your visit.  We will contact your insurance company for precertification of any scheduled procedures.  We will discuss your financial requirements due prior to the date of the appointment.   In addition to cash, we accept all major credit cards: VISA, MasterCard, Discover and American Express.

Insurance Terms

  • Deductible is a specified amount of money that the insured must pay before an insurance company will pay a claim. Each family member usually has a separate deductible.
  • Co-payments are flat fees charged each time you visit the doctor or use any medical service, regardless of the cost of the procedure.
  • Coinsurance requires you to pay a percentage of the total cost of the care. Many plans require either coinsurance or co-pay, but some charge both.
  • Maximum out of pocket expense is the most you’ll have to spend before all of your medical bills are covered.

Office Policies

We strive to make this a great experience.  In our effort to create a pleasant environment, we have instituted the following office policies:

  • Food and beverages are not allowed. All food and beverages must be consumed outside the office.
  • For parents with young children; for the safety of our staff and in consideration of all our patients, we ask that children are well attended during your visit.
  • In the exam room, visitors are limited to one adult per patient. All others will be asked to wait in the reception area.
  • Please silence your cell phone at check-in and upon entering our clinical area. Please be courteous to other patients while talking on the cell phone in the patient waiting room.
  • Payment is expected at the time of service unless prior arrangements are made. If you are unable to make your full payment, please ask to see or speak to the Office Manager prior to your visit.
  • There is a charge of $25.00 for completion of FMLA, Disability or Leave of Absence forms. Please allow a minimum of 5 business days turn around for completion.
  • There is a $30 charge for any appointment not cancelled at least 24 hours prior to your scheduled appointment.
